Design description
A questionnaire-based nationwide longitudinal prospective cohort study is conducted. Participants are recruited from 18 rehabilitation centres and hospitals in The Netherlands. 2000 participants with a physical disability or chronic disease will be followed during and after their participation in a tailored counselling programme. Programme outcomes on physical activity behaviour and patient as well as programme characteristics that may be associated with differences in physical activity behaviour after programme completion are being assessed. Data collection takes place at baseline (T0) and 14 (T1), 33 (T2) and 52 weeks (T3) and 6-8 years (T4) after discharge from rehabilitation.

Other inclusion criteria
Inclusion criteria were (1) aged 18 years or older; (2) having a physical disability and/or chronic disease; (3) receiving inpatient, outpatient or consultancy rehabilitation treatment at one of the participating rehabilitation departments or institutes; (4) participating in the RSE programme. Exclusion criterion were (1) unable to complete questionnaires, even with help; (2) participating in a PA programme other than RSE.
